The application consists of:
- Subinternship Rotation Application Form
- UCSF Student Clerkship Evaluation (Visiting students need an evaluation form from their school for UCSF to fill out)
- Health Requirement Form http://www.medschool.ucsf.edu/visitingstudents/applications/
- Application for an Elective (U.S. Student Application Form) http://www.medschool.ucsf.edu/visitingstudents/applications/
- 2” x 2” passport-sized photograph
- Check or money order for $150.00 per each 4 week elective made payable to the “UC Regents” in U.S. currency
- A letter of good standing signed by the Dean of the applicant’s school
- Completion of HIPAA training at home school or web-based module (module three) provided by UCSF at the following website (you will be asked to sign a certificate of completion on your first day of the rotation):

The requirements are:
1. Core Neurology: In order to be successful on the UCSF Department of Neurology clerkship/subintern rotations, you must complete a minimum of 4 weeks core neurology prior to the rotation.
2. Health Insurance: If you do not have personal health insurance coverage, you must obtain it from the Student Health Service, in Millberry Union, MU-H005, prior to the start of the elective. If you have questions about coverage, please call (415) 476-1281.
3. Malpractice Insurance: You must show proof that you are covered for Malpractice Insurance by your home school.
Minimum coverage: $1,000,000 per incident/ $3,000,000 aggregate.
4. Registration:
- You will arrive at 7:55 to join the Moffitt team at Morning Report on Monday. The location will be 505 Parnassus Ave., Room M-793. At this time, you will need to sign the Certificate of Completion for your HIPPA training.
- After Morning Report on the first day of your elective, you must register with the Office of Curricular Affairs, 513 Parnassus, S-221 (map of parnassus campus: http://www.ucsf.edu/maps/parnassus.pdf ). You will be issued a visiting student identification card that will give you access to computing facilities and the Milberry Union Fitness Center. Also, if you are working at either Moffitt Hospital or Mt. Zion Hospital, the Office of Curricular Affairs will assign you an ID# to access the patient database system.
- Once you have registered, you will need to get your visiting UCSF ID within 72 hours after registration.
